as used for functions and failures <\/a><\/span><\/span>.<\/strong><\/p>\n<p>Here and today we will talk about language to meaningfully formulate actions in the FMEA.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-7a821aa e-flex e-con-boxed elementor-invisible e-con e-parent\" data-id=\"7a821aa\" data-element_type=\"container\" data-e-type=\"container\" data-settings=\"{&quot;animation&quot;:&quot;fadeIn&quot;}\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-9e25cbd elementor-widget elementor-widget-text-editor\" data-id=\"9e25cbd\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>In most FMEAs we have 2 types of action:\u00a0<\/p>\n<ul>\n<li>Prevention Actions<\/li>\n<li>Detection Actions.<\/li>\n<\/ul>\n<p>If we use a tool as well designed to create a control plan in conjunction with the PFMEA then a third type will be needed:<\/p>\n<ul>\n<li>Reactions-Plan-Actions.<\/li>\n<\/ul>\n<p>\u00a0<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-292d4d4 e-flex e-con-boxed elementor-invisible e-con e-parent\" data-id=\"292d4d4\" data-element_type=\"container\" data-e-type=\"container\" data-settings=\"{&quot;animation&quot;:&quot;fadeIn&quot;}\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-a933a67 elementor-widget elementor-widget-text-editor\" data-id=\"a933a67\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>We formulate actions in the FMEA in order to get an effect:<br \/>Reduce the risk for a target to go wrong, for a described failure mode to become effective by effectively prevent this failure to happen or by detection of this failure, hindering to cause further damage.<\/p>\n<p>The prerequisite for such a risk mitigation is: The actions are written in a way to be <strong>clear and unmistakable<\/strong>.\u00a0<\/p>\n<p>My recommendation is to strictly address and use actions as prevention and detection as needed in a closed loop circle.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-488c907 e-flex e-con-boxed elementor-invisible e-con e-parent\" data-id=\"488c907\" data-element_type=\"container\" data-e-type=\"container\" data-settings=\"{&quot;animation&quot;:&quot;fadeIn&quot;}\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-f373e4c elementor-widget elementor-widget-text-editor\" data-id=\"f373e4c\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>To collect information, data, measurement values, may be to come to a decision, then I use <strong>detection actions (DA)<\/strong>.<br \/><br \/>In preparation of a design or before a process can start I need to take<strong> prevention actions (PA)<\/strong>. In the closed loop of a product development or a manufacturing process, <strong>prevention actions<\/strong> will control the process by use of the information collected by the detection action and help to keep the results stable.<\/p>\n<p>The <strong>Reac<\/strong><strong>tion-Plan-Action (RPA) <\/strong>\u00a0is only relevant in a process. We need to use DAs\u00a0 in the process as well. In most cases we use DAs for a decision: ok or NOK. For ok the process can proceed. For NOK a reaction is needed to keep the process going and to come back on track, to produce ok results as quick as possible. This type of actions will appear in the very right column of the production control plan.<\/p>\n<p>kommt nur in einem Prozess zum Einsatz. Auch im Prozess werden Entdeckungsma\u00dfnahmen verwendet. Period.\u00a0<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-873e947 e-flex e-con-boxed elementor-invisible e-con e-parent\" data-id=\"873e947\" data-element_type=\"container\" data-e-type=\"container\" data-settings=\"{&quot;animation&quot;:&quot;fadeIn&quot;}\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-5e8eb6b elementor-widget elementor-widget-text-editor\" data-id=\"5e8eb6b\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Tip #2:\u00a0 \u00a0Any kind of closed loop circle, what is a measurement of the current state plus the utilization of this information for a development loop or for process parameter improvement is a combination of a DA and a PA, two joint actions.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-8248021 e-flex e-con-boxed elementor-invisible e-con e-parent\" data-id=\"8248021\" data-element_type=\"container\" data-e-type=\"container\" data-settings=\"{&quot;animation&quot;:&quot;fadeIn&quot;}\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-8b57510 elementor-widget elementor-widget-text-editor\" data-id=\"8b57510\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Tip #3:\u00a0 \u00a0If it is a well working closed-loop circle the detection and prevention are of the same level of quality, to be rated with equal values for O and D. If either detection is bad or the the controller does not work properly then the closed loop circle will not deliver good results. In consequence we see the need for additional design loops, a non working design or too much rework and scrap in production.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-8756ba8 e-flex e-con-boxed elementor-invisible e-con e-parent\" data-id=\"8756ba8\" data-element_type=\"container\" data-e-type=\"container\" data-settings=\"{&quot;animation&quot;:&quot;fadeIn&quot;}\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-342a2d0 elementor-widget elementor-widget-text-editor\" data-id=\"342a2d0\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Let us come back to language.<\/p>\n<p>Actions are &#8220;instructions to do something&#8221;. <br \/>Therefore, actions should <strong>describe a doing<\/strong> in the text.\u00a0<\/p>\n<p><em>Which object needs to be handled in which way?\u00a0 <\/em><\/p>\n<p>\u00a0<\/p>\n<p>It needs a specific instruction.<br \/>The action will be assigned to a potential failure mode in order to prevent or detect exactly this failure mode.\u00a0<br \/><em><br \/>There may be lots of visual inspections in product development or process. &#8211; What exaclty is the matter, the way and the criterium to be inspected here ?<br \/><\/em><br \/>An action without responsible person and without due date typically will not happen.<\/p>\n<p><em>The definition who is facilitating this action until when is essential for success.<\/em><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-52e2023 elementor-widget elementor-widget-text-editor\" data-id=\"52e2023\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>How can we distinguish types of actions by phrasing?<\/p>\n<p>There are key words expressing what is the essential doing by this action.<\/p>\n<ul>\n<li>A\u00a0 <strong>detection action\u00a0 (DA)<\/strong> always includes a phrase\u00a0 \u00a0\u201eMeasure\u201c, \u201eTest\u201c, \u201eAudit\u201c, \u201eObserve\u201c, \u201eRecord\u201c, \u201eRelease\u201c, \u201eRate\u201c, \u201eInspect\u201c, \u201eGauge\u201c, \u201eFinal\u201c \u00a0as a noun or verb.<br \/>If such a key word is use in the action&#8217;s text then it must be a DA .<br \/>Virtual or real design verification or validation in design work is DA as well.<\/li>\n<\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-404756c elementor-widget elementor-widget-text-editor\" data-id=\"404756c\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<ul>\n<li>A Prevention Action (PA) typically is describing a preparation followed by something. Key words for PAs are \u00a0\u201edefine \u2026\u201c, \u201edetermine \u2026\u201c, \u201edescribe.. \u201c, \u201einstall\u2026\u201c, \u201eoptimize\u2026\u201c, \u201euse something \u2026\u201c, \u201enegotiate with someone about something\u201c, \u201edecide\u2026\u201c, \u201eintroduce\u2026\u201c, \u201eagree on \u2026.\u201c, \u201econtrol the process by \u2026\u201c.<br \/>During the development of the product (design) all activities to come to a determination and all preparations for the DV should be sorted in the type prevention action.<br \/><br \/>In process, the PA may be timed long before, e.g. as part of the equipment planning or as part of work preparation. Or it can be part of process control during the process. The PA will always be done before or latest during the process step, never afterwards.<\/li>\n<\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-2518ed9 elementor-widget elementor-widget-text-editor\" data-id=\"2518ed9\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<ul>\n<li>Reaction-Plan-Actions (RPA) will react on something. <br \/>Therefore, a proper phrasing includes a If &#8211; then &#8211; clause. This enables to react on different failure cases.<br \/>A deviation has been clearly detected by a detection action. <br \/>What you need to include into the RPA will strongy depend on your company regulation. <br \/>To control scrap material may be out of scope because it will no longer reach the customer. Any kind of rework or repair should be object of RPA workflow.<br \/>The difference between rework and scrap needs to be clear.\u00a0<br \/>Scrapped material must not go back into the material flow towards the customer. But it may be a major factor for process costs. My recommendation is to make scrap counting a part of the DA.<br \/>Or you need at least to track waste Was sie alles in die Reaktion mit einbeziehen, wird sich unternehmensspezifisch unterscheiden. Jede Art von Nacharbeit oder Reparatur ist daher hier von Bedeutung.<br \/>Auch die Unterscheidung zwischen Nacharbeit und Verschrottung. Ausgesondertes Material sollte nicht zur\u00fcck in den Warenstrom zum Kunden gelangen und kann einen wichtigen Kostenfaktor im Prozess darstellen. Allerdings sollte die Erfassung von Schrott schon Teil der Entdeckung sein.<\/li>\n<\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-ea43be2 elementor-widget elementor-widget-text-editor\" data-id=\"ea43be2\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><span style=\"color: #ff0000;\"><strong>Conclusions:\u00a0<\/strong><\/span><\/p>\n<ul>\n<li>After the analysis of potential failure modes the team is requested to get active. Actions = Doing.<br \/>FMEA actions are instructions to mitigate the risks.<\/li>\n<li>Actions list as well what has happend in the past as those activities ongoing or planed for future. Both needs to be part of the FMEA. It clearly describes the way to target. This is a major part of future lessons learned.\u00a0<\/li>\n<li>Types of action should be assigned clearly by key words or key phrases. Then you do not need to have discussion what is for prevention, what is for detection.<\/li>\n<li>Typically, you need a kind of close loop. The loop is formed by preparation\u00a0 and control loops in development and process, both given as PAs.<br \/>The loop is added by a measurement of current status (DA).\u00a0<br \/>These action bundles need to be planed and executed on identical quality level in order to properly work.<\/li>\n<li>By language, you enable people to perform what has been your mindset in planning. Without clear wording you create assumptions, hopes and wishes but not an effective risk mitigation.<\/li>\n<li>And actions need a stringent tracking.
